Reside in Seen Health's service area
Are 55 years old or older.
Require the nursing facility level of care, as defined by the State of California.
Are able to live in the community without jeopardizing the health and safety of yourself and others at the time of enrollment.

Review eligibility with our intake team
We will assess your healthcare needs and complete an evaluation to determine your eligibility.
We will submit documents to the state for approval.
Enroll in Seen Health
Once approved by the state, you will sign the enrollment agreement and review your personalized care plan.
We will enroll you in Seen Health! Your enrollment is effective the first day of the following month after you sign the Enrollment Agreement. For example, if you sign the Enrollment Agreement on March 14th, your enrollment will be effective on April 1st.
